Registration FAQs
Thank you for your interest in the Canton City School District.
Our district utilizes an online registration process. Please follow the steps outlined below. Don’t forget to check out the FAQs and links at the end of this page for additional enrollment information. If you need any assistance or have questions, please contact our Student Enrollment Center at 330-438-2525. Our registrars are happy to help you through the process.
Registration Steps:
Step 1: Click the link to Final Forms.
Step 2: Create your Final Forms account
- Under Parent, click on “New Account
- If you already have a child in Canton City Schools and need to add another child, log into Final Forms under the Parent tab with your existing email and password. Then, click on “Add Student” in the upper right corner.
Step 3: Complete each of the enrollment forms by filling out the required information and signing at the bottom. Forms will turn green to show they have been completed.
Step 4: Upload all of the required documents. Please refer to the document list below. Instructions on how to upload documents are included in Final Forms. However, if you are unable to upload them, please call the Student Enrollment Center at 330-438-2525 to schedule an appointment to bring in your documents. We are here to help.
Step 5: Review and sign your forms. Review all of your forms for accuracy, then sign and submit. You will receive a confirmation email from Final Forms.
*If you are currently homeless, living in a shelter, or sharing housing due to the loss of housing, economic hardship, or similar reason, additional enrollment support is available. Please call the Canton City Schools Homeless Services at 330-452-4363 for assistance.

Required Documents:
- Student birth certificate or passport
- Parent/Guardian driver’s license or state issued photo ID
- Immunization records
- Proof of residency - at least one of the following:
- Current mortgage, lease agreement, or real estate property tax bill in the parent/guardian’s name dated within the last calendar year
- A utility bill (gas, electric, water) in the parent/guardian’s name dated within the last 30 days
- A paycheck or pay stub in the parent/guardian’s name dated within the last 30 days and includes the current address
- A Notarized Residency Affidavit: This requires the homeowner/renter to provide the required proof of residency at the time of enrollment. It grants the enrolling parent thirty (30) days to provide the district with a valid proof of residency at that address
- Custody documents, if applicable
- Special education documents (IEP), if applicable

Ohio law requires that students must be 5 years old on or before August 25, 2026 in order to be eligible for kindergarten for the 2026-2027 school year.

Log into FinalForms
Click “Update Forms” under Actions
Review and update the information in FinalForms and sign the forms again.

Log in to FinalForms
Click “Update Forms” under Actions
Update the address and/or phone number under the appropriate contact and sign the form again.
NOTE: If you are updating your address, a new proof of residency must also be uploaded. See “What is Proof of Residency” under What Do I Need To Register a Student”

Go to FinalForms
Click the black ‘parent login” or Student login” button
Click on “Forgot your email?” or “Forget your password?”

The student’s legal guardian can register the parent. The legal guardian has the authority to make decisions about the person's health, education, and welfare, ensuring they are properly cared for and their needs are met. Refer to the link Enrollment and Custody Questions for specifics on who can register a student.

Log into FinalForms
Under Parent, click on “New Account”
If you have a child in Canton City and need to add another child, log into FinalForms under the Parent tab with your email and password. Click on “Add Student” in the upper right corner.

Students will attend the neighborhood school based on your address. Students are expected to attend their neighborhood school unless they are approved by the district to attend a Choice School. For more information on Choice Schools please refer to the link below or call Student Services at 330-438-2551.
